Personalized medicine includes the provision of comprehensive, coordinated, and integrated healthcare to an individual or a family on a 24 hours/7days /12 months/year with a response time not to exceed 15minutes. A team coordinator will be assigned to the individual or the family and will be on call with an assigned dedicated telephone number to respond to the medical needs of the patient, provide the necessary advice, and arrange for the service provision as per the request of the patient.
